Sec. 4-2.3. Environmental assessment

- (a) By September 1, 2016, all covered facilities must perform an environmental assessment of the facility using forms provided or approved by the department, unless an environmental assessment was performed on or after September 1, 2015.(b) Environmental assessments shall be updated annually and under the following conditions:(1) in the event that one or more cases of legionellosis are, or may be, associated with the facility;(2) upon completion of any construction, modification, or repair activities that may affect the potable water system;(3) expansion or relocation of a facility’s hematopoietic stem cell transplant and solid organ transplant units; or(4) any other conditions specified by the department.(c) The facility shall retain copies of the completed environmental assessment form in accordance with section 4-2.6 of this Subpart.
