Sec. 58-2.18. Records to be kept when blood or blood component transfusions are performed

The following information shall be included on the recipient's chart or in records maintained in the blood bank:

(a) date of the transfusion;

(b) name(s) of the person(s) who performed the transfusion and who attended the recipient during the transfusion;

(c) for each unit:

(1) blood component transfused;

(2) unit identification code;

(3) unit ABO and Rh groups;

(4) start time and completion time; and

(5) description of any adverse reaction and the results of related investigation;

(d) in the case of emergency issuance of uncrossmatched blood, the signature of the physician authorizing such emergency release.
