Sec. 17.4. Modifications to standard claim forms

(a) A commercial insurer or an article IX-C corporation may add to the claim form its own identification and similar information; however, no alteration may be made to the format of the standard claim form.
(b) Attachments to the standard claim forms by physicians and dentists, in lieu of completion of a portion of the claim forms, are permitted when the attachment is part of an office and bookkeeping system, provides information consistent with that required in the standard claim form, and is not found objectionable by the superintendent.
