Forms of applications for temporary licenses to be issued pursuant to Insurance Law section 2109 are prescribed for a temporary insurance broker’s license, insurance agent’s license, and title insurance agent’s license. These forms may be obtained upon request to the Department of Financial Services, Albany, NY.
