Forms of notice of termination of appointment of insurance agents and title insurance agents pursuant to Insurance Law section 2112 are prescribed as follows: for agents licensed pursuant to Insurance Law section 2103(a); for agents licensed pursuant to Insurance Law section 2103(b).
