Sec. 52.28. Medicare supplement insurance reporting form and refund calculation form

The data required by section 52.44(c) of this Part shall be submitted to the superintendent in the following form:
(a) Medicare supplement refund calculation form.

(b) Reporting form for the calculation of benchmark ratio since inception for group policies or certificates delivered or issued for delivery in New York State.

(c) Reporting form for the calculation of benchmark ratio since inception for individual policies delivered or issued for delivery in New York State.
