Sec. 702.2. Form of petitions

- (a) Petitions for declaratory rulings shall be in writing, signed by the petitioner or its attorney and addressed by certified mail, return receipt requested, to the Counsel, Department of Labor, Building 12, State Campus, Albany, NY 12240.(b) Petitions shall set forth the following:(1) a statement that a declaratory ruling is requested;(2) the name and address of the petitioner;(3) a full description of the facts and circumstances with respect to which a ruling is sought, including a complete description of any more extensive transactions and/or fact situations of which they may be a part;(4) citations to all the relevant statutory authority and regulatory provisions, whose applicability the petitioner is seeking to have determined; and(5) a full disclosure of the petitioner's interest.(c) Accurate copies of all relevant documents and supporting materials shall be submitted with such petitions.
