Sec. 595.7. Functional program

  • (a) A functional program shall be developed and submitted with the provider's application for initial certification. The functional program shall, at a minimum, include:
    (1) a description of the program mission, and the population to be served;
    (2) admission and discharge criteria;
    (3) a description of the specific service needs of the defined target population;
    (4) a description of the existing service system, including but not limited to, mental health, health, social and other support services;
    (5) a description of the residents' utilization or non-utilization of the existing service system and the rationale for such service use patterns;
    (6) a description of the goals and anticipated outcomes of the program, including the anticipated average length of stay for residents;
    (7) a description of the provider's role, if any, in providing a crisis bed capacity within the geographical area;
    (8) a description of the proposed staff pattern, including job descriptions and qualifications, number of FTE's for each position, the supervisory structure and a staffing schedule;
    (9) a description of the on-site services which will be provided to the residents, including how these services with further the goals and anticipated outcomes of the program;
    (10) a description of how the residential program will appropriately utilize community- based services;
    (11) a description of the expectations, if any, which may be placed upon the resident regarding program participation within the residence and participation in programs outside the residence; and
    (12) a description of the process for providing of information about and explaining the rights of residents to both staff and residents.
    (b) The functional program shall serve as a basis for delineating anticipated program outcomes which shall be monitored pursuant to section 595.13 of this Part.
