Sec. 18.5. Number plates and ownership documents

- (a) Number plates.Local authorities shall remove and store number plates for 30 days, after which such plates must be destroyed or surrendered to the Department of Motor Vehicles. A list of all destroyed plates, with the name of the last owner of the vehicle, must be submitted monthly to the Department of Motor Vehicles, Division of Data Preparation and Control, Empire State Plaza, Albany, NY 12228.(b) Prior ownership documents.If the registration, title or other ownership documents are in the possession of the local authority, such local authority must destroy them or surrender them to the Department of Motor Vehicles, except that a certificate of title must be surrendered with form MV-907A, if such form is used.(c) Form MV-906A.(1) Form MV-906A must be serially numbered and printed by the local authority.(2) The form must be 7⅜ long by 3¼ high (standard punchcard size).(3) Format of Form MV-906A:(i) Front.SERIAL NUMBERNAME OF LOCAL AUTHORITYCERTIFICATION OF ABANDONEDVEHICLE ACQUISITIONVEHICLEYEAR MAKETYPE COLOR(S)CONDITIONIDENTIFICATION NUMBER(GOOD -- FAIR--POOR --SHELL)(VIN)NAME AND ADDRESS OF OWNERNAME AND ADDRESS OFLIENHOLDER(S) (IF APPLICABLE) MV-906A(ii) Reverse.OWNER NOTIFICATION(S) —LIENHOLDER NOTIFICATION(S) —DATE(S) — TIME(S) — BYDATE(S) — TIME(S) — BYI certify that the information on this form iscorrect and true.SignatureTitleMV-906A
