Sec. 55.1. Verification and contents of petition

- (a) Applications shall be verified by the person designated to exercise supervision over the destruction of records (see section 152.5 of regulations to govern the preservation of records [Part 152 of this Title]) or, in the absence of such a person, by the principal accounting officer of the petitioner, and shall set out in detail the nature of the papers, records, books, invoices, stubs, maps or documents which it wishes to destroy and the necessity for such action.(b) The application shall show:(1) whether the papers to be destroyed are originals or copies;(2) whether or not they directly support financial records;(3) the subject to which they relate, classified as nearly as possible in accordance with regulations adopted by the commission to govern the preservation of records; and(4) the period of time to which the records apply.
