Each exhibit shall contain:
(a) a title page showing:
(1) the applicant's name;
(2) the title of the exhibit; and
(3) the proper designation of the exhibit; and
(b) if consisting of 10 or more pages, a table of contents citing, by page and section number or subdivision, the component elements or matters contained in the exhibit.
