Part 345. Tax Refund Offset for Persons not Served by the State Child Support Enforcement Program |

- A new written application is required for each year services are desired. No later than September 30th of each year, for inclusion in the tax refund offset process during the next succeeding calendar year, an applicant seeking services under this Part shall provide to the department a completed application, in the form prescribed by the department, and the following information and documentation:(a) a certified transcript of a money judgment for a sum certain for arrears accrued under an order of support;(b) a sworn statement that the order of support is no longer subject to appellate judicial review and that the sum set forth as uncollected on the judgment is accurate;(c) the name and address of the applicant; and(d) the name, last known address and social security number of the respondent whose tax refunds are to be intercepted.
