Sec. 346.3. Statement of respondent

(a) When an order of support is payable through the SCU, the respondent shall provide the SCU with the following information:
(1) name;
(2) address of residence;
(3) social security number;
(4) date of birth;
(5) place of employment; and
(6) such other data as may be determined by the department to be pertinent.
(b) The respondent shall be informed of the payment regulations of the SCU.
