Sec. 474.3. Loss, mutilation or destruction of permits, plates and stickers

If a permit is lost, mutilated or destroyed, the State Tax Commission will issue a duplicate upon application therefor and payment of a fee of $1. Similarly, in the event of the loss, mutilation or destruction of a plate or sticker, a substitute one will be issued upon application therefor and payment of $2. Application for a duplicate permit or substitute plate or sticker should be made on form TMT-334.
