- (a) Before exercising the functions of judicial hearing officer, a person so designated shall take and file with the Chief Administrator of the Courts a sworn statement that such person will faithfully and fairly do such acts and make such determinations and reports as may be required by the designations as a judicial hearing officer. The statement shall be in the form as set forth in subdivision (b) of this section.(b) Sworn statement of hearing officer.I, __ (give full name), do solemnly swear (or affirm) that I will support the Constitution of the United States, and the Constitution of the State of New York, and that I will faithfully and fairly do such acts and make such determinations and reports as may be required by the designation as a judicial hearing officer, according to the best of my ability.(Signature)Sworn to before me thisday of, 19.Notary Public
