FORM S-3 TEMPORARY CONFINEMENT ORDER STATE OF NEW YORK □ SUPREME COURT □ COUNTY COURT PART: __ COUNTY: __________ IN THE MATTER of A Temporary Confinement Order Pursuant to CPL 330.20 in Relation to __________ Defendant __________ An application having been filed with this court by □ the District Attorney □ the Commissioner of Mental Health, pursuant to the provisions of subdivision 14 of CPL 330.20, for a recommitment order commiting the above-named defendant to the custody of the Commissioner of Mental Health for confinement in a secure facility for care and treatment for six months from the date of the order, And, the above-named defendant having been ordered to appear before this court for a hearing to determine whether he has a dangerous mental disorder, And, the above-named defendant having failed to appear in court as directed, And, this court having issued a warrant pursuant to the provisions of subdivision 14 of CPL 330.20 directing an appropriate peace officer to take the above-named defendant into custody and bring him before the court, And, the aforementioned warrant having been executed and the above-named defendant now being before the court for a hearing to determine whether the said defendant has a dangerous mental disorder, NOW THEREFORE, it is hereby ORDERED that the above-named defendant be confined in the following institution pending further order of the court: [Here name an appropriate institution located near the place where the court sits and which has been designated by the Commissioner of Mental Health as suitable for the temporary and secure detention of mentally disabled persons.
