F.C.A. §§ 528, 533 Form 5-9 (Remand Commitment Undertaking) 7/2003 FAMILY COURT OF THE STATE OF NEW YORK COUNTY OF ________________ In the Matter of a Paternity Proceeding Docket No. Petitioner, REMAND COMMITMENT –against– (Undertaking) Respondent. ________________ TO ANY PEACE OFFICER OF THE STATE OF NEW YORK AND TO THE SHERIFF of the JAIL OF __ COUNTY: The above-named respondent having been brought before this Court upon a petition filed under Article 5 of the Family Court Act, seeking a declaration that said Respondent is the father of a certain child born out of wedlock and other relief; AND the said Respondent having thereupon been duly held for further proceedings to be held on , he is required to furnish an undertaking in the sum of $ with sufficient surety approved by the Court, for his appearance on that date and in default thereof he is hereby committed to the custody of the above-named Sheriff or Warden until said date unless sooner discharged according to law; AND YOU ARE, THEREFORE, COMMANDED forthwith to take charge of the above-named Respondent and deliver him into the care and custody of the said Sheriff or Warden.
