Sec. 149-1.5. Annual report

  • (a) Each school district and community school district in the city of New York that is required to submit a comprehensive district plan to the commissioner pursuant to section 149-1.3(a) of this Part shall submit to the commissioner an annual report in a form and at a time prescribed by the commissioner. Such report shall provide data, individually for each school providing services in accordance with this Subpart, and shall include, but not be limited to, the following:
    (1) the unduplicated number of pupils in each grade who received compensatory instruction and services in accordance with the provisions of this Subpart in mathematics, reading, and writing in the base year with subtotals for pupils of limited English proficiency;
    (2) the total number of years individual pupils have received remedial instruction at the conclusion of the base year in accordance with the provisions of this Subpart and in accordance with the provisions of other State or Federal programs;
    (3) the number of pupils, by grade, who received instruction and services in accordance with the provisions of this Subpart in the base year and who will be retained in the same grade in the current year;
    (4) expenditures in the base year from the setaside for pupils with compensatory educational needs, and expenditures from other State and Federal compensatory programs as determined by the commissioner; and
    (5) an evaluation of the results of the execution of the district plan and an evaluation of each schoolwide project.
    (b) Each school district that is required to submit a district plan to the commissioner pursuant to section 149-1.3(b) of this Part shall submit to the commissioner a complete annual report in a form and at a time prescribed by the commissioner. Such report shall include, but not be limited to the following:
    (1) a description of the expenditure of the base year's setaside for compensatory education for eligible pupils;
    (2) the number of pupils with compensatory educational needs served in the programs and how they were identified as pupils having such needs; and
    (3) an evaluation of the results obtained from the operation of such programs.
