- As used in this Part, the following definitions shall apply to the terms listed below:(a) Case management shall mean a process which includes gathering information about and from residents, making assessments, setting goals, evaluating progress, documenting information, screening residents, making referrals and preparing appropriate forms and reports.(b) Counseling shall mean a structured intervention in an individual or group setting to assist residents to understand themselves and their interactions with their environment, to address their identified needs, to set personal goals, and to develop skills to achieve these goals.
