Sec. 177.15. Notification to ombudsmen of critical incidents

- (a) The facility or program director shall immediately notify the office of the ombudsman of any critical incident involving a youth under his/her care.(b) For the purpose of this Part, a critical incident shall mean:(1) an incident which indicates that a violation of rights has occurred that could lead to disciplinary action and/or criminal prosecution;(2) an incident in which a youth has died or sustained substantial physical injury; or(3) an incident which could give rise to a claim against a youth.
