Sec. 2603.2. Application submission requirements

(a) The applicant must complete such forms and attachments as prescribed by the division.

(b) Essential components of the application include:

(1) documentation supporting the eligibility of both the applicant and the neighborhood;

(2) general goals in effecting neighborhood preservation, stabilization or improvement;

(3) quantified, measurable objectives to be accomplished by the completion of the activities proposed in the work plan;

(4) budget, specifying the use of funds requested from the division and other financial resources, including the one-third match requirement and staffing pattern; and

(5) documentation of community support.
