PURSUANT TO THE PROVISIONS OF THE State Administrative Procedure Act, NOTICE is hereby given of the following action:

Action taken:

Repeal of Appendix 1; and amendment of section 15.1(c) of Title 14 NYCRR.

Statutory authority:

Mental Hygiene Law, sections 7.09, 29.01 and 31.04

Subject:

Repeal of outdated forms and conforming amendments.

Purpose:

To eliminate antiquated forms.
