- (a) The agency shall maintain appropriate and accurate administrative, financial and student records, as well as any other records that may be required by the commissioner. The agency shall maintain the following records for each student enrolled:(1) registration form;(2) test data;(3) attendance data;(4) diagnosis of needs;(5) prescription of activities; and(6) record of progress and achievement.(b) The agency shall submit required reports as prescribed by the commissioner.
